The First Visit
Before Dr. Maidlow has you scheduled, you will be asked to request an appointment on the portal. New patient intake forms will be emailed to you directly to be filled out at your convenience online. Be prepared to discuss your health history, current needs, and goals. Complete the medical intake form as thoroughly as possible. A "Informed Consent" agreement will be provided via email and must be signed before treatment begins.
A confirmation will be sent via email and/or text once the intakes are completed. Please arrive 10-15mins before your scheduled appointment.
To prepare for your initial visit with Dr. Maidlow, please wear comfortable, loose clothing. He will provide a comprehensive exam and if no red flags are found, Dr. Maidlow will treat you at this first visit. Please allow about 45mins-1hour.

Do I Have to Go Forever?
It’s a commonly held belief that once you start going to a chiropractor you have to go for the rest of your life. That’s just not true. “I always ask patients what their goals are for care and aim to help them meet those goals. I never have someone commit to chiropractic care for the rest of their life,” says Dr. Maidlow. He typically will want establish care by seeing a patient 1-2 time a week for 2-4 weeks, unless the injury or pain deems for more frequent care. It is his goal to see patients on maintenance care once every 1-3months. The other option is you can book an appointment with him as needed.
